Frederick A Slezak MD MBA
Frederick A. Slezak, MD, MBA is an Ohio resident graduating from The University of Akron and medical school at The Ohio State University College of Medicine. He is certified in Cannabis Science and Medicine by the University of Vermont. Dr. Slezak is also certified by both the American Board of Surgery and the American Board of Colon & Rectal Surgery. He received his Certificate to Recommend (CTR) from the State Medical Board of Ohio in December 2019 just after the registry opened. He has been active at the state and national level ever since giving lectures on cannabis medicine and currently is an instructor at the Cleveland School of Cannabis. Dr. Slezak has evaluated and recommended medical cannabis for hundreds of patients in the Ohio program.
